Metastatic Prostate Cancer Causing Chronic Subdural Hematoma

Abstract
The authors describe a case of metastatic prostate cancer to the leptomeninges causing chronic subdural hematoma. A 57-year old male was transferred to our hospital with progressive decline in consciousness. Computed tomography(CT)scan revealed a low-density layer over the left frontal convexity that was thought to be consistent with a chronic subdural hematoma. The patient was taken urgently to the operating room and a left frontal burr hole was performed. Through the burr hole, soft tissue masses were observed at epidural space and removed for the histopathological examination. Histopathological examination confirmed the metastatic prostate cancer but no tumor cells within subdural fluid. The magnetic resonance imaging demonstrated leptomeningeal carcinomatosis. Whole brain radiation therapy was undertaken. Since his guardians wished no further treatment, he was transferred a local hospital where his death was claimed 8 months after that. The pathogenesis of the subdural hematoma in this case might be due to secondary to preexisting chronic subdural effusion by involvement of arachnoid granulations at left hemisphere by the metastatic prostate cancer. Although it is unrelated to patient¡¯s survival rate, subduro-peritoneal shunt could be considered as a treatment option for the quality of life.
